Premier Therapy Associates (“Premier Therapy,” “we,” “us” or “our”) provides speech, physical and occupational therapy in Idaho Falls and Rexburg, Idaho. This Privacy Policy explains what information we collect when you visit premiertherapyidaho.com, how we use it and the choices you have.
Information you give us. When you fill out a form on our website, such as a contact form, screening or appointment request, or the milestones guide download, we collect the information you enter. We provide therapy for children, but our website is intended for parents, guardians and adults. We do not knowingly collect personal information online from children under 13. If you believe a child has submitted information to us, please contact us and we will delete it.
